Space Rental Agreement:
Space rental is $50./ hour for OPAL members and $60./ hour for non-members. The first hour fee is due in advance to hold the date, the remaining balance is due at the beginning of the event.
A 30 day notice of cancellation is required to receive a full refund. Less than 30 days notice will result in the loss of $50 deposit. A $25 transfer fee will be charged if the event is rescheduled. In the event that OPAL cancels the event date a full refund of the deposit will be credited.
Unless otherwise determined, event guests will have access to the front courtyard, gallery, studio, second classroom, hallways, bathrooms, and kitchen.
Artwork and art materials are not to be disturbed at the event. If a piece of exhibited artwork is damaged or stolen during the event, the event contact person will be held responsible to pay the artist’s value of the work within 30 days of the event.
An Oak Park Art League representative will be present during the event to act as site contact.
The OPAL site contact will be present and available to open the space 1 hour before the event and 1 hour after the event for set up and clean up.

Waiver and Release:
In consideration of my attendance at or use of Oak Park Art League (“OPAL”) facilities, I hereby assume all risks of personal injury, death, property loss or other damages that may result from or arise out of attendance at or use of or participation in any of OPAL’s programs or activities. I understand that the foregoing waiver of liability on my behalf shall apply to any and all claims against OPAL and its officers, directors, employees, servants, agents or affiliates (“OPAL Affiliates”) for any such personal injuries, property loss, or other damages connected to or arising out of any of the aforesaid risks.

I represent to OPAL that I am able to perform those activities which I may undertake at OPAL and that I am solely responsible for all health risks association with such activities.

I hereby, on behalf of myself and my heirs, executors, administrators and assigns, fully and forever release and discharge OPAL and OPAL Affiliates, and of each of them, from any and all claims, damages, demands, rights of action, present and future, known or unknown, anticipated or unanticipated, resulting from or arising out of my attendance at or use of OPAL facilities, or my participation in any of OPAL’s activities or programs, including those which arise out of the negligence of OPAL and OPAL Affiliates from any and all liability for any loss, or theft of, or damage to personal property.

I acknowledge that I have carefully read this Waiver and Release and fully understand that it is a waiver and release of liability. Participant’s legal guardian MUST sign waivers if Participant is under 18 years of age.
